Registered Nurse - Home Visit RN

The Home Visit RN will support clients and caregivers throughout our Queens, NY territory with our Preferred Home Health Care business (a Help at Home company).
A typical day for a Home Visit RN includes:
- Making the initial in-home evaluation visit and regularly reevaluating the patient’s nursing needs
- Initiating the plan of care and necessary revisions
- Preparing clinical and progress notes for each patient visit and summarization of care conferences on his/her patients in a timely manner as per Agency policy
- Informing personnel of changes in the condition and needs of the patient
- Counseling the patient and family/significant others in meeting nursing and related needs
- Processing orders and notifying physician of patient needs and changes in condition
- Completing certification/recertification orders and discharge summaries
- Determining the amount and type of nursing and personal care support needed by each individual patient
- Providing supervision and education to Personal Care Assistants
- Participating in peer review and Quality Assessment and Performance Improvement as assigned
- Meeting quality, productivity, and compliance requirements
- Performing other related duties as assigned
- Graduate of an accredited nursing school
- One (1) year of clinical nursing experience, preferably in home health/home care setting
- New York or multi-state RN licensure
- Experience working with medically fragile clients, addressing medical, social and behavioral components of care
- Ability to exercise initiative, independent judgment, and work independently to leverage community resources that benefit our clients
- Excellent interpersonal, written and verbal communication skills
- Ability to thrive in a fast-paced and ever-changing environment

- Demonstrated critical thinking and decision-making skills
