Help at Home is hiring an IDD Program Specialist!
Job Summary:
The Program Specialist provides management and oversight for direct support services delivered to young adults and adults with Intellectual and Developmental disabilities (I/DD). The Program Specialist also works with the collaborative care team to advocate for individuals with I/DD, maintains compliance with all Federal, State and County regulations, and accurately documents consumers' progress.
Essential Duties/Responsibilities:
- Assist clients with I/DD and/or Autism in developing goals and strategies to best service each individual according to their Individual Support Plan (ISP).
- Manages and advocates for clients and directly supervises all Direct Support Staff.
- Trains Direct Support Professionals on agency policy, the implementation of ISPs, and on behavioral management techniques.
- Assists the consumer with attaining goals in order to have positive outcomes as described in the individual's ISP.
- Monitor individuals' progress and services delivery through on-site visits and data collection.
- Travels to and participates in monthly team meetings, ISP meetings, Annual Review Updates and other team meetings as necessary.
- Respects the autonomy and right to confidentiality of all consumers and families.
- Produces accurate documentation of the services. Documents care provided and response of the consumer/family. Submits time sheets and all other documentation in a timely manner.
- Advocates for changes in service(s) as necessary.
- Performs all duties in an ethical manner and in compliance with local, State and Federal laws, Medicare/Medicaid regulations and established personnel policies and procedures.
- Performs other job-related duties as assigned.
Education and Experience:
- Associates degree and three (3) years of experience in the human services field; OR a Bachelor's degree and one (1) year of previous experience.
- Supervisory experience preferred.
- Personal vehicle which is insured and meets state safety/licensure requirements.
Required Skills and Abilities:
- Excellent organizational skills; ability to multitask and manage multiple responsibilities.
- Strong problem-solving skills.
- Strong communication skills, oral and written.
- Excellent interpersonal skills.
- Proficient with Microsoft Office Suite, especially Excel.
Travel Requirements:
- Regular travel on a daily or weekly basis required, even in inclement weather.
Physical Requirements:
- Ability to move, transport, or position up to 50 pounds.
- Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
- Ability to communicate effectively and clearly with others to exchange information.
