As a Home Care LPN working in our Waycross, GA territory, you will work one-on-one with an individual in their home and assist them with tasks such as (but not limited to) assessments, vital signs, tube feedings, medication administration, and activities of daily living. Most importantly, you will act as an advocate for your client(s) by educating them and their family members on best care practices and collaborating with interdisciplinary care teams to achieve continuity of care.
Other responsibilities for this position include:
• Adherence to client care plan/485
• Care documentation, client observations and outcome evaluation
• Working closely with Help at Home clinical team and client PCP as needed to address clients' medical needs
Preferred Skills/Abilities:
• Excellent verbal and written communication skills
• Able to establish and maintain good working relationships with client, client’s family and administrative staff
• Adequate computer skills to document electronically on a tablet
• Ability to work independently and as part of a broader care team
Qualifications:
• Current Georgia or multi-state LPN license
• Current CPR certification
• One (1) year of clinical nursing experience
