Job Summary:
The main focus of this position is to assist the Qualified Developmental Disability Professional (QDDP) in the coordination and implementation of services with referral source, consumer and employee, ensuring the delivery of quality services. This occurs through the direct supervision of employees, supports for consumers, and ongoing communication. All coordination must be conducted within contract, state and federal regulations, as well as Company Policies and Procedures.
Essential Duties and Responsibilities:
- Assist the QDDP with the Person Centered Planning process, including assisting with the development and implementation of the Person Centered Plans (PCPs) and the Individual Support Plan (ISPs)
- Assist with the design and implementation for the consumer’s support and assisting with teaching strategies targeted at assisting consumers with reaching their outcomes as outlined in the consumer’s ISP
- Assist with supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies
- Assist the QDDP with the revision of the PCP and ISP outcomes as necessary based on data collection, feedback from staff, and direct observation of the consumer. Outcomes may only be changed with approval from the consumer, the consumer’s guardian, and members of the interdisciplinary team.
- Assist the QDDP to ensure that services and supports are provided in the PCP and ISP.
- Assist the QDDP in completing the monthly outcome report and provide to the Case Manager.
- Maintenance of consumer’s file including, but not limited to:
- Consumer profile data with emergency contact numbers.
- Copies of Medicare/Medicaid cards for all supported living sites, current Release of Information forms, Consent forms, and current signed Consumer Rights statement.
- Copies of guardianship paperwork if applicable.
- Financial records using the Help at Home Inc. financial system including documentation of all account spending, personal spending, and household spending.
- Daily progress notes, monthly reviews, and other consumer related documentation.
- Medical information including documentation of necessary medical appointments, lab results, immunizations, physician’s medication orders, Medication Administration Records, and controlled drug count sheets.
- Monthly Health Summary.
Required Skills/Abilities:
- Excellent organizational skills; ability to multitask and manage multiple responsibilities.
- Strong problem-solving skills.
- Excellent interpersonal skills.
- Strong communication skills, oral and written.
- Proficient with Microsoft Office Suite.
- High School Diploma or GED required.
- One year (1) of experience in the field of developmental disabilities.
- Supervisory experience preferred.
- Personal vehicle which is insured and meets state safety/licensure requirements.
- Valid driver's license required.
